Ongoing Care Monitoring & Case Management
If you can’t be there due to distance, let us be here for you and them.
We will monitor the person you are caring for on a regular or as-needed basis. We become your local “surrogate family caregiver” when you can’t be there due to distance or other committments.
Our on-going case management and support is tailored to the unique circumstances and needs of the family and senior. It might include visiting with the senior weekly or monthly, monitoring the care plan, reminder phone calls, monitoring home support workers, liaison with other service providers, going to health care appointments and providing documentation and follow-up. We report regularly to family members and any other members of the care team.
Ongoing Care Monitoring & Case Management Activities
A Navigating Eldercare 101 session is highly recommended first, prior to purchasing a monthly package.
Our monthly packages can assist and support seniors & family caregivers with:

- On-going case management, observation and assessment of elder(s)

- Act as a liaison to oversee the care of an elder(s) for out of town families; assist in system navigation

- Help setting goals to remain independent and engaged in meaningful activities

- Assisting with transition from home and a retirement community, assisted care facility, or long term care

- Coordination of health care services & referrals into community services

- Coordination of transportation services and prescription management

- Screening, arranging and monitoring in home help; caregiver training

What’s Included in Ongoing Care Monitoring & Case Management
- Any of the activities listed above
- On-going visits and phone calls fir observation and assessment of client and any other agreed upon activities including companionship/visitation, goal setting, health care advocacy, coordination of appointments & calendar
- Detailed documentation after each visit with any noted changes or concerns directly sent to family members
Monthly Packages
Happy to customize and adjust – but trying to get away from watching the clock for both our sake and yours. Predictable bills and visits flexible for the needs of that day to stay longer if required or leave earlier if a nap is calling. It will all work out in the wash!
- 1 visit each month to check in, problem-solve, support & reporting (peace of mind for family members living outside the Comox Valley)$250/mo
- 1 visit each week plus 1 appointment each month $1200 + GST
- 1 half visit (1.5-2 hours) each week plus 1 appointment per month $700 + GST
- 4 visits monthly (full morning or afternoon) support & report plus 1 appointment (Doctor, Specialist, hair or nails) and one family caregiver support call: $1300 + GST
