It is often confusing, complicated and challenging for family caregivers and seniors to understand the ins and outs.

Our Navigating Eldercare 101 session helps by:

• Listening to concerns and challenges and objectively identifying and prioritizing the main issues
• Working with your family to build a roadmap and plan for the person being caring for and next steps. This often includes talking about getting the care you need from the healthcare system, housing options and aging in place strategies
• Practical advise on how to have conversations with family members, the person needing care and health care professionals
• Giving resources and places to get more information
• Building your confidence and knowledge as a caregiver
• Reducing your stress

What’s Included in our Navigating Eldercare 101 Session

• Review of intake information prior to the session
• 60-minute consultation in office, telephone or web-based video conference
• 15 minute follow up report via email with key points from consultation and next step items
• A copy of “Get Your Eldercare Affairs in Order”
