Many of us don’t know who to call to find assistance to help ourselves or an aging parent one manage at home or when it is time to move. It is often confusing, complicated and challenging to understand the ins and outs.

Seniors and caregivers often experience stress and feelings of being overwhelmed. Especially when the bottom falls out or when there isn’t a plan in place.

 We have customized services to help you, regardless if you are a long-distance caregiver, or a seasoned caregiver experiencing new challenges or a senior facing an age-related transition.

Navigating Eldercare 101

Navigating Eldercare 101 sessions valuable one to one assistance to family caregivers and seniors to identify and stabilize the current eldercare situation, assist with problem-solving and an outline a roadmap for next steps and action items.                                                                     PACKAGE rate $299

 

Personalized Eldercare Planning

When important decisions need to be made or you are facing an eldercare crisis, it’s easy to become overwhelmed and unsure of how to find the right solution or put in a care plan.   We follow Teepa Snow as a Certified Consultant to use her Puzzle pieces to identify changes and best meet needs of People living with dementia. We work together to find the best way to meet needs. As an RN with gerontology certification I am happy to meet to advise, guide and consult with families and seniors through a more complex eldercare challenge or transition.                                                          CONSULTATION $106/hourly

 

On-going Monitoring & Case Management

This service is for families who live long distance from the person they caring for in the Comox Valley. We assist and support seniors and family caregivers with on-going case management, observation and assessment of the older adult; medical advocacy; coordinate health care and social services, act as liaison to oversee the care of an elder for out of town families.

Our monthly reports update general activities and needs but we connect directly for changes or needs. Our Nurses are all registered and as such we do not charge GST. Our invoices may be used for Disability tax claim or some insurances cover Nursing services.                                                                                                                                                                                                                        $70/hour for ongoing CV clients

NEW – Occasional Comox Valley clients Ride and Nurse accompany to GP/Specialist appointments 

We are often asked to give a ride to a Dr appointment for those outside our clients. Now we offer to have a Nurse help by taking notes before the appointment to focus the Dr visit as well as write their recommendations to take home and/or update the family to be involved in healthcare decisions. For those requiring wheelchair accessiblity we would book a wheelchair taxi on your behalf. $106/hr plus km
